Is it possible to "fill and save" in today s economy? It is if you are talking about the "fillable" PDF forms available in the Gaming Regulation section of the Department of Justice website.
How does this save you time? First, fillable forms are more convenient because you can fill them in by typing on your computer. Second, if you save the completed form, any changes or future versions can be generated by simply updating the initial version.
All Gambling Control Division forms are available to download, fill in, save and print. Fill-and-print forms include:
Combined Gambling and Liquor License Application (form 5)
Card Room Contractor (form 9)
Personal History Statement (form 10)
Non-Institutional Lender (form13)
Card Game Tournament (form 14)
Manufacturer/Distributor
/Route Operator (form 17)
Forms cannot be submitted on-line. They must be printed and delivered or mailed to: Gambling Control Division, Department of Justice, 2550 Prospect Ave., P.O. Box 201424, Helena, MT 59620-1424.
To use a fillable form:
1. Go to

2. Find the form you are looking for and click on the PDF link.
3. Click the computer disk icon (which reads Click to save this file to your computer or another location ) near the top of the page and save a copy of the form to your files.
4. Open Adobe Reader, find the PDF on your desktop or wherever you saved it and double click on it. The PDF will open in Adobe Reader.
5. Type in your information. Save frequently by going to File > Save.
If you need several versions of the same form, go to File > Save As and rename the file. This way, if you have to fill out the form for several businesses, you don t have to retype your general information.
